Project Failure Files: No Self-Preservation Skills

In Episode 42 of the Project Failure Files weekly webcast, our focus was “Forgetting to CYA,” in which Sharon and I discuss the importance of proactive documentation in workplaces where accountability is often replaced by blame. Recognizing the red flags of a toxic culture—such as excessive bureaucracy, unclear roles, and leaders who avoid responsibility—can help professionals understand when to adopt a structured approach to keeping records. The discussion highlighted best practices for maintaining a paper trail, including summarizing conversations via email, tracking key decisions, and using project management tools to ensure transparency without unnecessary overhead.

Beyond documentation, the episode explored the importance of presenting evidence effectively when deflecting unwarranted blame. Knowing when to escalate an issue versus diplomatically addressing conflicts can safeguard reputations and maintain team cohesion. The discussion also covered strategies for staying professional while asserting accountability, including constructive communication and fostering a culture where documentation is seen as a best practice rather than a defensive mechanism.

In the conclusion, we emphasized that while those environments where “CYA” is necessary can hinder collaboration, clear and transparent record-keeping protects individuals and project success. Listeners are encouraged to evaluate their documentation habits, adopt best practices for accountability, and use documentation as a tool for clarity rather than mere self-preservation.

Enjoy the episode!
